Mental Health Assistant

The Pediatric Psychiatry Collaborative is an innovative grant program designed to ensure screening, assessment, diagnosis, and treatment of children presenting in pediatric primary care settings with behavioral health disorders. This is a key position in a state-wide initiative to expand mental health and care coordination to support pediatricians. This program is composed of pediatrician screening and assessment, education, parent guidance on behavioral health issues, education for pediatric providers/staff at the practice sites, and integrated care management between pediatricians, child and adolescent psychiatrists, and community mental health agencies.
Responsibilities: 
Work in collaboration with Clinician and Child Psychiatrist to provide care management resources to pediatricians and families Assist with identifying behavioral health resources in the community Facilitate calls with pediatricians and pediatric office staff  Input patient referral information into EMR Conduct follow up calls with patients/families to confirm linkage to services and/or address barriers  Participate in scheduled Hub team meetings  Participate in NJAAP/MHC sponsored webinars and/or trainings  Assist with promoting Morris/Sussex HUB services Other duties as assigned  Qualifications: 
Bachelor's Degree in Psychology, Counseling or a related Human Services Area  Excellent communication skills  Strong organizational and time management skills Ability to work in an interdisciplinary team setting  Computer literate with proficiency in Microsoft Office and EMR
