Job Summary: Are you passionate about making a difference in the lives of individuals with intellectual disabilities? We are currently seeking a dedicated and compassionate Qualified Intellectual Disabilities (QIDP) Case Manager to join our team. As the QIDP Case Manager, you will play a pivotal role in overseeing the development, implementation, and monitoring of individual support plans (ISPs) for individuals with diverse needs. Your advocacy and leadership will ensure the provision of active treatment in alignment with regulatory standards and the mission of our facility. If you possess excellent communication skills, a strong commitment to advocacy, and the ability to coordinate interdisciplinary teams, we encourage you to apply for the Intellectual Disabilities Case Manager position.
Key Responsibilities: ISP Development and Advocacy: Lead the interdisciplinary team in the creation, revision, and approval of individual support plans. Serve as the primary advocate to ensure active treatment is provided based on regulatory requirements and the needs of individuals. Monitoring and Implementation: Monitor the progress of ISPs and ensure the organized delivery of services by coordinating staff schedules, completing required documentation, and providing necessary training. Compliance and Quality Assurance: Recommend revisions and address deficiencies in services to maintain compliance with facility policies, state regulations, and federal standards. Coordination and Documentation: Monitor and document supports/plans, and coordinate activities of all disciplines involved to facilitate the implementation of active treatment. Resource Management: Direct work, review and approve expenditures, and recommend policies to ensure a safe and well-equipped living environment for individuals. Communication and Collaboration: Maintain open communication and provide updates regarding changes in condition or behavior to parents, guardians, and court officers. Promotion of Welfare and Dignity: Assist individuals in advocating for their concerns, understanding their rights, and making informed choices. Community Integration: Support individuals in establishing and maintaining meaningful relationships with family members and community members. The Ideal Candidate Will Have: Bachelor's degree in Human Services field (e.g. Sociology, special education, rehabilitative counseling, psychology); one year of experience working directly with clients, with intellectual disabilities or other developmental disabilities; strong advocate for providing clients with learning opportunities through education, experience, and exposure; possesses strong, supportive, positive, and respectful communication skills, including the ability to provide clear guidance and instruction; experienced in Patient Care Technician (PCT) practices; organized, with skills in attention to detail and strong accountability for timelines; demonstrates positive and productive facilitation skills; experienced in conducting both individualized and group training sessions; maintains awareness of services through policies, rules, and regulations; exhibits leadership skills conducive to effective teamwork; able to encourage self-motivation. Knowledge of:
developmental disabilities and their ramifications; treatment standards of accrediting bodies; current trends in the care and training of the developmentally disabled population; regulations governing the care and habilitation of people with developmental disabilities. Ability to:


coordinate and integrate multidisciplinary services; express information regarding people clearly and concisely to staff; collect and analyze data regarding the effectiveness of supports/plans; apply current trends in the care and teaching of people with developmental disabilities; establish and maintain effective working relationships; maintain the highest standards of confidentiality. This position is eligible for Veterans' Preference per ARSD 55:10:02:08.
