In this role, you'll play a crucial part in providing supportive services and counseling to individuals enrolled in healthcare and home care programs. As a valued member of our interdisciplinary team, you'll assist patients, clients, residents, and their families or caregivers in addressing social, emotional, and economic concerns, empowering them to achieve and maintain their optimal level of functioning.
Responsibilities:
Offer supportive counseling and crisis intervention to individuals and families facing medical challenges, ensuring they understand the complexities of their situations.
Coordinate and plan healthcare programs, collaborating with patients, families, caregivers, and healthcare teams to ensure comprehensive care.
Respond promptly to suspected cases of abuse, neglect, or violence, adhering to ethical guidelines and legal requirements.
Develop individualized care plans by connecting patients and families with appropriate resources and services from social, health, and human services agencies.
Provide referrals, current information, and educational resources to promote informed decision-making and access to available programs and services.
Participate actively in care conferences and transitional rounding, advocating for patients and demonstrating professionalism at all times.
Utilize critical thinking and problem-solving skills to address complex situations and contribute effectively to the healthcare team.
Establish rapport and maintain effective communication with patients, families, caregivers, and healthcare communities.
Depending on departmental needs, may provide social services for donors and transplant recipients.
Qualifications:
Bachelor's degree in Social Work from an accredited curriculum is required. (Note: Prior to January 1, 2009, a bachelor's degree in Social Work or a related field was also acceptable.)
Experience in healthcare and/or mental health hospital settings is preferred.
Basic Life Support (BLS) certification may be required within six weeks of employment, depending on location and position.
Must hold a license in good standing in the state(s) of practice, as follows:
In Iowa: Licensed Bachelor Social Worker (LBSW)
In Minnesota: Licensed Social Worker (LSW) or equivalent as per Minnesota statute
In North Dakota: Licensed Baccalaureate Social Worker (LBSW)
In South Dakota: Social Worker license (SW)
GSS locations: Licensure or certification as a social worker as required by the state's licensing board.
