$55,000 to $65,000 Per Year You'll Be Responsible For: Overseeing the work of the Social Service Designee Helping residents and families adjust and solve problems regarding their social, emotional, and financial needs Assisting with Room and Roommate Changes Attending Medicare Meetings Discharge planning for patients by arranging for follow up community services, home health referrals and alternative placements, Advocating for residents and families regarding resident rights issues.
