Responsibilities
• Develop relationships with residents and their families using a consistent caregiver approach.• Assess, observe, and evaluate resident's clinical condition and document findings.• Update or modify care plans as needed.• Administer medications accurately and in a timely manner.• Participate in meetings related to resident outcomes and care.• Respond to residents, family, or responsible parties in a timely and professional manner.• Participate in the review of clinical indicator reviews.• Provide care and services to residents.• Maintain a clean and organized work environment.• Comply with all policies, procedures, and regulations.
Requirements
• Graduate of an approved Licensed Practical/Vocational Nursing program.• Passion to care for seniors and advocate for residents.• Experience with electronic medical records preferred.• Possess good verbal and written communication skills.• Ability to work well in a team.
