The Role Under general direction, the primary function of the Health Care Coordinator is to provide coordination of medical care for clients for designated agency programs. He/She will provide services relating to the promotion of wellness for clients through educational lectures, individual counseling, and special topic seminars. Additionally, he/she will support providers in the provision of care including recording vital signs, administering methadone, discussing chief health concerns, performing testing, and assisting with procedures and documentation. Assesses, plans, evaluates , and delivers nursing care in collaboration with RN and/or Physician. This work is carried out in support of the mission and goals of Samaritan Daytop Village.
What You Will Do Assists with patient care including checking vital signs, blood draws, obtaining other specimens for laboratory analysis and culture as ordered by the medical provider, discussing chief health concerns, and administering injections and immunizations. Observes, measures, records, and reports data relating to patient mental and physical status. Educates and encourages clients with medical conditions to adhere to medication regimens. Provides group and individual health education to clients. Identifies and develops with the Director of Nursing a curriculum of health education specific health education topics, such as smoking cessation, tuberculosis, sexually transmitted diseases, hepatitis, family planning, pregnancy, asthma, and diabetes that are coincidental to disease management of persons served. Provides gender-specific health education, such as female health education (e.g., anatomy, physiology, and gynecological issues) and male health education (e.g., prostate screening). Develops programs that provide information relative to general health concerns to promote well-being for identified medical needs of agency clients. Develops, assesses, obtains, and disseminates health-related information for clients. Maintains Standard Precautions and as needed educates clients and staff in Standard Precautions. Assumes responsibility for maintenance (security/safety of medication storage) and general appearance/hygiene of the medication administration unit/area. Documents all assessments, interventions, and responses to treatment in progress notes of the clinical record following agency procedures for timely and complete documentation. Who You Will Be NYS LPN license. BLS certification. High School Diploma or Equivalent. Graduate of a formal Technical or Vocational LPN Program. At least One (1) year experience as an LPN working with clients with Substance Use Disorder (SUD) and/or Serious Mental Illness (SMI) or in a similar working environment. Proficiency in Phlebotomy and EKG. Available to work a flexible schedule as needed in response to participant and staff needs. Computer literacy including proficiency in Microsoft Office Suite and EHR. Ability to put patients at ease and explain physician's instructions. Ability to multi-task with excellent organizational skills. Ability to maintain the confidentiality of patient records. Experience working directly with people from diverse racial, ethnic, and socioeconomic. backgrounds. Knowledge of medical terminology. Team player and ability to work independently.
