The Licensed Practical Nurse (LPN) is a member of our Health and Wellness Team and provides wellness education, collaboration, and training under the direction of a Registered Nurse (RN) for people receiving Community-Based services and the employees who support them. The Licensed Practical Nurse (LPN) is a hybrid position- working in homes conducting home visits to collect health status data and report changes to the Registered Nurse (RN), training staff on medical procedures, and providing consultation over the phone on the health needs of those supported.
The Licensed Practical Nurse (LPN) complies with the MN Nurse Practice Act, regulatory standards and relevant laws.
As the Licensed Practical Nurse (LPN), you will create an environment that supports wellness and to advocate and collaborate in health care related to the health services for people receiving supports and services from Mains'l.
Some core responsibilities of the Licensed Practical Nurse (LPN): Assist in facilitating generalized and individualized education and training sessions to build and maintain competence of the Mains'l direct support employees and management in ensuring the health needs of the people we support are met Consult with the Registered Nurse (RN) and management team to provide needed education when medical status' change Active participation in Health and Wellness Team Meetings and work groups Assist in the enrollment process of persons coming to Mains'l for 24 hour supports or hourly supports with significant medical needs Review health care provider referrals to assist with coordination of healthcare It is important to us that the nurses who join our team are : Resourceful, networking with a variety of resources and educating self and others Coachable and wanting to engage in lifelong learning and development Committed to partnering and collaborating Knowledgeable and caring Dedicated to healthy and safe community living of those we serve and ourselves that is based in what is important to people Responsible in effectively managing their time; identifying needs, balancing multiple requests Skilled at effective communication using everyday language Able to maintain the balance of what is important to us and for us as an agency and as individuals Continuously making the world a better place!
