Qualifications: Current Licensed Practical Nurse (LPN) in the State of LouisianaLong term care experience requiredMedical facility auditing experience requiredKnowledge of electronic charting systemSelf-starter and the ability to recognize issues and solve themStrong computer, medical records, communication and customer service skillsGreat attention to detailJob Duties: Monitor medication administration to ensure that patients are taking their medications accuratelyReport any hazards or concerns to the DON or other appropriate staff memberParticipate in QAPI initiative to improve clinical processesReview medical records to identify treatment plans or process errors or problemsPrepare reports on patient outcomes that are used to improve patient care and nursing practicesWork with clinical staff to develop, implement, monitor and update quality improvement initiativesDevelop and implement a system for monitoring the quality of care delivered by nurses and other staff colleaguesObserve patients to identify any changes in their condition or behavior that may indicate a need for interventionsAudits throughout the facility will be requiredEnsure all admits and readmit orders are correctConfirm all resident appointments are scheduled timelyMonitor all immunizations, including checking the links system and ensure all immunizations for new admits comply with guidelinesMake rounds to ensure nursing processes are being carried out effectivelyEnsure IDT is supported with clinical decisionsMonitor acute outings monthlyAll other tasks assigned by DONExceptional People providing Exceptional Care!
