Are you passionate about making a difference in people's lives? We are seeking a compassionate and skilled Behavioral Health Care Coordinator to join our team. In this hybrid role, you'll have the opportunity to work remotely and onsite, collaborating with individuals, families, and our community partners to address behavioral health needs effectively.
Key Responsibilities:
Assess, plan, and implement tailored services to meet the behavioral health needs of individuals.
Coordinate with internal and community resources to provide seamless support across transitions of care.
Identify and eliminate barriers to accessing necessary services.
Manage cases with a low to moderate level of service intensity, ensuring comprehensive care management interventions.

Experience/Education/Qualifications:
Master's degree in Mental Health, Social Work, Clinical Psychology, or a related field required.
Current unencumbered Oregon credential/licensure in LCSW, LPC, PhD or PsyD in Psychology, CSWA, LMSW, or LMFT.
Minimum of one year of clinical experience required.
Experience or training in health care delivery systems, managed care patients, and computer applications including electronic documentation preferred.
Knowledge/Skills/Abilities:
Excellent written and verbal communication skills, with the ability to engage effectively with members, providers, and organizational staff.
Proficiency in motivational interview techniques and conflict resolution.
Knowledge of State and Federal (HIPAA) laws pertaining to confidentiality of protected health information.
Strong critical thinking skills with the ability to identify complex problems and develop effective solutions in a member-centric manner.
Social perceptiveness and the ability to work with a diverse population, demonstrating personal sensitivity and a non-judgmental attitude.
Understanding of Medicare and Medicaid rules and regulations, as well as health plan benefit structure and policy.
