The purpose of the IRTS and Med Clinic Nurse position is to oversee psychiatric and medical services for IRTS clients, and facilitate Med Clinic services, under the supervision of the prescribing Psychiatrist or Physician Assistant. The IRTS Nurse also focuses on maintaining the safety of clients in the IRTS program. This position is primarily administrative in nature, with ability to apply nursing skills of phlebotomy, injections, obtaining vitals and assessments, and medication oversight. The person in this position needs to be independent, self-directed, and able to develop creative systems or innovative techniques to meet the needs of the clients and staff. The IRTS Nurse generally makes decisions that affect the day-to-day operations of the program. This position interacts with internal contacts and external contacts (pharmacies, law enforcement, clinics, etc.). Essential Job Functions Provides nursing services for clients in the IRTS program (25%) Provide a health screening for each client within 72 hours of admission into the program Develop a system for ongoing monitoring and addressing the health needs of clients Provide client education (individual or group) regarding disease prevention, health care and maintenance, medications and management Address any special health care needs of the clients Respond to the needs of clients with co-occurring substance use disorders Ensure referrals to community psychiatric and medical services are coordinated and occur in a timely manner, including arranging for annual physical exams for clients as needed Monitor effectiveness of medication and evaluate medication side effects Consults and advises program staff regarding health and medical care of clients (25%) Develop and implement guidelines for program staff regarding when to inform the nurse of client health concerns and how to obtain medical care for clients Answer occasional question or inquiry from staff during off hours if they are confused about a medical order or medication Consult and advises program staff on an ongoing basis regarding issues such as disease prevention, health care and health maintenance Ensure effective and prompt response to staff to medical emergencies, including those related to intoxication and withdrawal Share information, knowledge and ideas with fellow team members Seek information, support and clarification from team members as needed Follow agreed upon interventions and/or program plans Assist in problem-solving team, client and program issues Assist other team members in the delivery of needed services to clients Participate in weekly team meetings and/or ancillary meetings Maintain up-to-date knowledge and understanding of current developments and issues in the professional nursing and mental health fields Participates in weekend on-call rotation for crisis consultation Leads Tasks Wellness Committee Perform monitoring and supervision duties as defined by Minnesota Statutes 148.171–148.285 Provides appropriate documentation based on external and internal requirements (20%) Check physical exam form and provider's orders for completeness at time of client's admission Check and corrects any discrepancies between the information on the forms and the client's medication bottles Ensure that medical and health documentation is accurate, thorough, and maintained properly Develop policies, procedures, and forms to ensure and document all nursing services Review policies and procedures related to nursing services at least annually and provides updates as needed Serves as the Medication Clinic Coordinator for IRTS clients (15%) Coordinates bi-weekly medication clinics and follows through with provider orders for IRTS clients: Track medication clinic schedule Track all scheduled lab work and medication injections, and ensures they are completed or administered as ordered Ensure client's chart is up-to-date in the areas of medication clinic notes, lab reports, and any pertinent medical reports prior to each medication clinic Communicate with pharmacy to resolve any prescription filling issues that arise Communicate with medical labs regarding test orders and lab results as needed Follow up with medical providers and clinics as needed Communicate with consulting psychiatrist as needed to ensure proper administration of medications Provide oversight of the IRTS facility's Medication System: Oversee the organization of the Medication System Ensure all medication records are accurate and complete Count medications weekly and as needed; ensure each client has enough medications on an ongoing basis Monitor and replenishes first aid supplies Report problems with Medication System to Treatment Director and Clinical Director Ensure medications are administered saftey and accurately, including establishment of methods for the following activities: Staff-supervised, peer-monitored self administration of medications Determine when and how staff are to inform the IRTS Nurse or physician of problems or concerns with staff-supervised, peer-monitored self administration of medications including issues such as failure to administer medications, refusal of medication, adverse reactions to medications, errors in administering medications Provide training for staff that are responsible for the staff-supervised, peer-monitored self administration of medications including direct observation of staff in order to evaluate their competency prior to supervising peer-monitored self administration independently Coordination of Medication Clinics and follow through with provider orders for non-IRTS clients (15%) Track medication clinic schedule and communicate/participate in clinics as required Partner with Fairweather Coordinators and Case Managers in collecting and entering lab data Communicate with pharmacy to resolve any prescription filling issues that arise Communicate with prescribing psychiatrist and medical providers/clinics, as needed Ensure client's chart is up-to-date in the areas of medication clinic notes, lab reports, and any pertinent medical reports prior to each medication clinic Actual percentages may vary slightly depending on jobsite location and assigned coverage Physical Requirements of Position Standing Occasionally

None Qualifications Education and Experience Required Current license to practice professional nursing in Minnesota Meet or exceed qualifications of a mental health practitioner in Minnesota Statutes 245.462 Bachelor's degree in nursing Valid driver's license and an insured vehicle Preferred Three to five years of experience in the nursing field, preferably with adults diagnosed with a serious and persistent mental illness Knowledge of psychiatric medications and other psychiatric treatments Fluent in a non-English language or competent in the culture of an ethnic group to which 20 percent or more of the agency's recipients belong Knowledge Areas Psychology – Human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research messages; and the assessment and treatment of behavioral and affective disorders Sociology and anthropology – Knowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, cultures and their history and origins Clerical –Administrative and clerical procedures and systems such as word processing, managing files and records, designing forms, and other office procedures and terminology Law and government – Laws, legal codes, court procedures, precedents, government regulations, executive orders, agency rules, and the democratic political process Safety and security – Relevant equipment, policies, procedures and strategies to promote effective operations for the protection of people, data, property, and institutions Medicine –Information and techniques needed to diagnose and treat human injuries, diseases, and deformities.
