Responsibilities
• Conduct screening/intake interviews for program admission• Determine level of care recommendation• Enter screening/post-screening follow-up contacts into database• Complete required documentation• Provide referrals to applicants needing crisis or community-based referrals• Follow up with applicants on eligibility/wait list• Analyze admissions regarding census and referral sources• Provide outreach and educational services to the community• Conduct psycho-educational lectures and therapeutic groups• Act as liaison/advocate to patients' families and community agencies
Requirements
• High School Diploma or GED• At least 1 year experience conducting Substance Use screenings/intake or substance abuse counseling• Good working knowledge of community social service resources• Ability to articulate agency's eligibility criteria and treatment continuum of care• Knowledge of co-occurring disorders• Knowledge of Client Rights and Code of Ethics• Computer literacy, proficiency in Microsoft Office Suite and EHR• Experience working with diverse backgrounds
