Class Title: Human Service Program Specialist Base Salary: $36,178.71 – 39,795.71 Location: Florida Department of Health in Seminole County 400 W. Airport Blvd Sanford, FL 32773 This position requires you to have a valid Florida Driver license or the ability to obtain one within 30 days of hire. This position may also require you to use your personal vehicle if a state vehicle is not available. This highly confidential and specialized position dedicated to providing and assisting clients with community referrals, resources, and linkages to HIV/STD testing, treatment and case management of HIV, Syphilis, Gonorrhea and Chlamydia. Provide HIV/STD testing and counseling, assist with specimen collection and processing for STD screening; provide PrEP/PEP education and referrals for target populations; assist with data collection and data entry; organize and participate in outreach events. Promote health education events, workshops, conferences, and presentations; establish, build, and maintain community relationships and partnerships to increase community awareness of services offered at DOH-Seminole and the STD program. Support community initiatives outside of DOH-Seminole; develop relationships with community-based organizations, groups, faith-based organizations, leaders, and private medical providers. Coordinate community engagement events in collaboration with other community organizations with a health and wellness focus; participate in the development of culturally sensitive, age-appropriate HIV/AIDS, STD, and other health promotion materials and outreach programs. Review and update referral and resource materials; develop and organize promotion of programs for linkage to care for people who test positive for HIV at DOH-Seminole, and other local providers. Support Surveillance staff when needed to process labs and to communicate with PMD's for medical information and education. Duties and Responsibilities include, but are not limited to: Personal management of HIV/STD caseloads Conducting interviews and re-interviews of persons infected with HIV/STD's Conduct field investigations to locate patients and partners who are infected with Syphilis, HIV, Gonorrhea and Chlamydia, and refer them for examination and treatment. Perform venipuncture in the field, clinic, and anonymous test sites. Provide pre and posttest counseling for HIV. Attend scheduled Community Events. Required Knowledge, Skills, and Abilities: Knowledge, skills and abilities, including utilization of equipment, required for the position: Knowledge, skills and abilities, including utilization of equipment, required for the position: effective interpersonal skills; computer software applications. Skill in: effective written and verbal English communication. Ability to: perform basic computer operations; successfully complete venipuncture, HIV/AIDS 501 and Introduction to Sexually Transmitted Disease Intervention (ISTDI) training within 6 months of employment. .
Qualifications: Minimum – Current Florida Department of Health Employee with STD/Surveillance experience .
