Total Compensation Estimator  Your Specific Responsibilities: This is work investigating the distribution and determinants of disease prevalence in populations to determine and describe means for detection, prevention, intervention and control of health problems. Working under the direct supervision of a Frontline Supervisor, the incumbent serves in a developmental capacity, providing direct disease and public health awareness intervention activities under the auspices of the Sexually Transmitted Disease (STD) Program, including but not limited to Partner Services (PS), epidemiological cases analysis and investigations, notification, Pre-Exposure Prophylaxis (PreP) referrals, counseling and referral services of individuals exposed to or at-risk of infection from Chlamydia, Gonorrhea, Hepatitis, Herpes Simplex Virus (HSV), HIV/AIDS, Human Papilloma Virus (HPV), Syphilis and other bacterial, viral or biological agents. Incumbent will also be responsible for linkage to care and re-engagement to care activities. Direct guidance and instruction on work methods, procedures, and techniques are provided. All activities are performed observing federal, state, department and CHD Information Security and Confidentiality statutes, rules, regulations, policies, Technical Assistance Guidelines (TAGs) and Internal Operating Procedures (IOPs).
Coordinates and conducts timely personal interviews and re-interviews of patients infected with HIV for: (1) educating patients regarding their infections and appropriate medical care; (2) developing plans to locate and refer their sex and needle sharing partners and others at risk for infection to a medical facility for examination and appropriate treatment. Performs cluster interviews on persons determined to be potential information sources for known cases.
Performs rapid/timely field follow-up to locate and refer infected persons, sexual and needle sharing partners, as well as others who are at risk, to an appropriate diagnostic facility for examination and treatment. Educate, and counsel high risk individuals about Pre-Exposure Prophylaxis (PreP). Collection of specimens by venipuncture, oral swabbing and/or urine-based techniques. Link newly identified HIV patients and previous positive returning to care to Test & Treat Providers in the community. Adhere to the locally established T&T procedures.
Performs case management activities as directed in the process of discovering and/or identifying infected individuals. These activities will include but are not limited to work prioritization and records documentation and management, clustering, HIV pre- and post-test counseling, visual case analysis and epidemiological techniques, data entry and/or other related activities. Utilizes STARS and HIV/AIDS Linkage Module to ensure Linkage to care (LTC) and STD cases are entered in accurately and timely.
Locates and links clients infected with HIV to medical treatment services as well as any other services needed. Provides coordination, guidance, and assistance in the accessing medical, social community, legal, financial, employment, vocational and/or other needed services. Non-medical case management duties including but not limited to: Initial comprehensive assessment of service needs. Timely and coordinated access to medically appropriate levels of health, prevention and support services and continuity of care. Assures and verifies linkage to specialized health education programs and medical care for HIV. Assures that patients lost to care are identified, contacted on a timely basis after referral, linked to care with a medical provider of client's choice, and followed up as necessary to ensure engagement with HIV medical care on an ongoing basis.
Assists supervisor in the completion of monthly team statistical data, maintenance of logs & files. Incumbent is expected to cooperate with supervisory staff in allowing quality assurance reviews as needed in the incumbent's/county vehicle, in the field or clinical setting.
Provides supports services not considered routine or expected duties that facilitates HIV/STD control efforts and Emergency Operations functions required of the incumbent will include but are not limited to; Special Needs Shelter, Red Cross shelter, and other related work as required.

Required Knowledge, Skills, and Abilities: Ability to establish and maintain effective working relationships with others. Ability to communicate effectively, both verbally and in writing and through empathic listening and to elicit information as appropriate to the needs of the audience. Ability to listen to and understand information and ideas presented through spoken words and sentences. Asking questions as appropriate. Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition and grammar. Knowledge of principles and processes for providing customer and personal services including needs assessment techniques, quality service standards, alternative delivery systems, and customer satisfaction evaluation techniques. Ability to persuade others to change their minds or behavior. Ability to plan, organize and coordinate work activities. Ability to operate commonly used office equipment such as computers, printers, fax machine and paper shredder. Ability to use Microsoft Office products (Outlook, Excel, Word, PowerPoint, knowledge of Access database software is a plus). Knowledge of business and management principles involved in strategic planning. Ability to multi-task and handle face-to-face public interactions. Ability to educate patients. Qualifications: Minimum – Two years of experience working with a disease control program, social services program, or community-based organization. Preferred - Incumbent in this position is preferred to be bilingual in English/Spanish or English/Creole. Where You Will Work :
