The purpose of the Housing Stabilization Coordinator is to connect individuals experiencing housing instability or homelessness to the appropriate housing stabilization services. Responsibilities will include providing Housing Consultations, as well as direct support to individuals experiencing homelessness at the Northeast Outreach and Opportunity Center (NOOC).
This is a position is partially funded by a Minnesota DHS Office of Economic Opportunity Emergency Services Program (OEO ESP) grant which runs through June 2021. Grant funding may be available to extend ESP services beyond the end date, and other opportunities within Tasks' may become available to continue employment. Essential Job Functions Provide trauma-responsive housing and mental health support to NOOC visitors (45%) Conduct initial intakes and orientation to the center, building trust and rapport with new visitors to encourage further engagement Collect demographic, outcome, and other relevant data required for HMIS, grant funding, and annual reports Complete required documentation, including but not limited to: intake forms/assessments, individual support plans, case notes, data collection for the Homeless Management Information System (HMIS) Provide services, such as general treatment support and service coordination for necessary mental health, housing, and other applicable services through Tasks and/or other agencies in a timely manner Help individuals identify and reach emergency shelter, connect with the Coordinated Entry system, and learn about and connect with other housing options including mainstream housing resources or housing for those with a mental illness or other disability Conduct housing consultation services for eligible NOOC visitors. Establish and maintain necessary professional relationships with other community resources, agencies, and service providers as directed by supervisor Provide Housing Stabilization Services for individuals, including consultation, transition, and sustaining services (45%) Develop a housing focused person-centered plan based on assessment outcomes Supporting the person in identifying their strengths, needs and wants in housing including cultural requirements and/or preferences Supporting the person to make an informed choice in their housing transition/sustaining provider Offering resource information for services that support non-housing related goals as identified in the person centered planning process Coordinating with other service providers currently working with the person Helping the person understand their rights to privacy and appeal information Annually reassessing the person-centered plan as it relates to housing. Assist clients in locating suitable housing in the community Assist client in locating community resources Performs outreach to the community and other appropriate agencies regarding Housing Meets regularly with Clients to discuss progress, problems, and plans Take clients for tour to potential sites Assist clients in completing housing application Performs other related functions and duties (10%) Participate in staff and program team meetings, as scheduled; participate in staff committees as time allows Maintain knowledge and understanding of current developments and issues in homelessness, mental health, and housing resources Support Tasks Unlimited fundraising, promotional, and family events Actual percentages may vary slightly depending on jobsite location and assigned coverage Physical Requirements of Position Standing Occasionally

None Qualifications Education and Experience Required Be able to pass a criminal background check A knowledge of local area housing resources A knowledge of harm reduction, housing first, and other homelessness issues Experience meeting people where they are and providing trauma responsive care Valid driver's license and insured vehicle Preferred Associate's degree in Human Services or related field and 2-3 years of experience working with the homeless, or 4 years in a related field, such as mental health, corrections, affordable housing, or chemical health Fluent in a non-English language or competent in the culture of an ethnic group to which 20 percent or more of the worker's recipients belong Knowledge Areas Psychology – Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation; psychological research messages; and the assessment and treatment of behavioral and affective disorders Therapy and counseling – Knowledge of principles, methods, and procedures for diagnosis, treatment and rehabilitation of physical and mental dysfunctions, and for career counseling and guidance Sociology and anthropology – Knowledge of group behavior and dynamics, societal trends and influences, human migrations, ethnicity, cultures and their history and origins Customer service – Knowledge of principles and processes for providing customer and personal services.
