Applicants for this position must be located in Carlton County. Family Rise Together (FRT) is Black-led nonprofit organization based in Duluth, MN, supporting all people, including the Black, Indigenous, and People of Color (BIPOC) community. FRT is a provider of Housing Stabilization Services, a Minnesota Medical Assistance benefit to help seniors and people with disabilities, including mental illness and substance use disorder, find and keep housing. The Housing Navigator will provide Housing Stabilization Services in Carlton County, MN primarily, but may also work with some clients in St. Louis County. Three services are provided within Housing Stabilization Services.
Housing Consultation – service to develop a person-centered plan. Housing Transition – service that supports a person to find housing. Housing Sustaining – service that supports a person to maintain housing. To qualify for this position, candidates must agree to complete and pass a Department of Human Services background study and sexual exploitation screening. The ideal candidate for this opportunity will: Complete relevant training. Have knowledge of housing supports and other resources in and around Carlton County, MN. Have a valid driver's license and acceptable driving record and be able to drive to meet with clients daily. Be at least 18 years of age. The essential functions of the Housing Navigator include, but are not limited to the following: Provide housing searches, advocacy, and support to locate, sustain, or transition to housing. Assisting low-income families in applying for federal, state, and local housing programs such as Section 8 or public housing. Assist individuals with disabilities, including mental illness and substance use disorders, in finding suitable, affordable housing that meets their needs. Work with individuals to identify and resolve any issues that may arise with their current housing situation. Provide culturally and trauma-informed responsive services to individuals and/or their families in the home or community. Design and implement housing plans and other required documentation in a timely manner. Inform clients on housing related issues, such as landlord/tenant disputes and discrimination in housing. Coordinate with other service providers in order to ensure that individuals receive all the help they need. Conduct home assessments to determine if their current living situation is safe, healthy, and accessible. Assist individuals to develop a plan that addresses housing needs and desires over time. Help identify financial resources available through federal and state government assistance programs, community organizations, and other resources. Assist with collecting assessment data through interviews and 1:1 interactions. Maintain client confidentiality and must comply with HIPAA laws. Ensure all assessments, plans, and documentations are completed and entered into the electronic tracking system in a timely manner. Maintain minimum billable hours weekly and document it within the electronic tracking system. Coordinate services with other programs and providers as needed. Attend individual, team, and staff meetings and employee training as required. Networking and outreach to promote programming. Ability to demonstrate culturally competent practices. Provide empathetic listening and validation. Minimum Qualifications (Knowledge, Skills, and Abilities) This position requires local and county-wide travel using a personal vehicle on a regular basis. The desired relevant experience for this position includes two years of experience working in the nonprofit, social services sector; however, candidates with any combination of experience including lived experience that is likely to provide the knowledge, skills and abilities to be successful in the position will be considered. Commitment to Family Rise Together's mission, understanding of social work best practices, and experience with homeless and/or poverty-related services. Experience working with individuals experiencing homelessness, and with people experiencing mental health and substance use related conditions. Knowledge of issues related to housing/homelessness and other community resources. Candidates should have solid communication and organizational skills and the ability to work well independently and as part of a team. Excellent oral and written communication skills, including computer skills. The ability to observe and assess without judgement or bias. Passion to serve clients from diverse backgrounds and experiences. Interpersonal skills, including the understanding and respect for boundaries, values, and personal differences. Organization and time management skills. Problem solving and solution implementation skills. Flexibility and adaptable to changing conditions in the workplace. COMPENSATION: This is a rewarding opportunity offering an hourly pay of $19.00 to $21.00 per hour, based on years of experience. This position also offers health benefits, paid time off, and paid holidays.
