Responsibilities
• Rounding with providers to discuss potential referrals for Hospice Savannah Inc.• Educating referral sources, patients, and families about hospice home care and inpatient services• Reviewing all clinical records and reviewing payer sources• Scheduling visits when appropriate• Demonstrating the Hospice Savannah core values and compliance with service excellence standards• Modeling the Hospice Savannah Corporate Values• Maintaining proficiency in Word, Excel, Outlook, and general office equipment• Interacting and communicating with staff, patients, families, physicians, medical office staff, and the community• Providing excellent customer service and demonstrating analytical and critical thinking skills• Possessing access to a personal vehicle and an operable cell phone
Requirements
• Graduate of an accredited school of Nursing, Licensed Practical Nurse preferred but not required, with experience in hospice, oncology• High School diploma or equivalent required for non-clinical roles• Experience in healthcare, business development, outreach or related fields preferred• Knowledge of ICD 9/10 coding and/or medical billing experience preferred• Minimum of three years' work experience in a healthcare environment• Experience in Medical Terminology and accurate data entry and typing skills• Excellent verbal, written, and computer skills• Proficiency in Word, Excel, and Outlook• Working knowledge of general office equipment• Strong time management and communication skills• Excellent customer service, analytical and critical thinking skills• Physically capable of lifting up to 20-75 lbs with proper body mechanics• Ability to bend, twist, stoop, sit, and walk for extended periods• Ability to type and assist with general office work• Excellent communication and problem-solving skills• Professional demeanor
