We are currently seeking a qualified and professional Chaplains to join our Home Healthcare agency. Our Home Healthcare agency services areas across all counties of the Bay Area. We have a current need for Chaplains who are eager and able to work in the Bay Area. We provide a comprehensive care program to each of our patients. Requirements include: Must have graduated from an accredited school with degree in social work. Current CPR certification. Develop a plan of care for each assigned patient. Assesses patient needs, plans for, evaluates and modifies care to meet goals of social work interventions. Collaborates with all disciplines to plan and evaluate team goals for each patient.
