Full job description Position Summary
The aide is a trained worker who, under the supervision of appropriate professional staff, provides supportive services that assist the patient or family in the achievement of physical and emotional comfort.
Position Accountabilities
Maintains established departmental policies and procedures, objectives, quality assurance program, safety,
environmental, and infection control standards.
Utilizes appropriate technique in personal hygiene of client.
Utilizes safe transfer techniques and ambulation of client.
Reports and documents changes in client's condition.
Follows the "Plan of Care" designated for client.
Completes all in-services as indicated by supervisor.
Reads and records temperature, pulse blood pressure and respirations.
Utilizes basic infection control procedures.
Understands basic elements of body functioning and changes in body functions that must be reported to the supervisor.
Maintains a clean, safe and healthy environment for the patient.
Recognizes emergencies and implements appropriate emergency procedures as applicable.
Understands the physical, emotional and developmental needs of and ways to work with the populations served by NHS Sequoyah Home Health, including the need for respect for the patient, his or her privacy, and his or her property.
Utilizes appropriate and safe techniques in personal hygiene and grooming that include: bed bath, sponge, tub or shower bath hair shampoo in sink, tub or shower, nail and skin care, oral hygiene, and toileting and elimination.
Communicates effectively with all types of patients.
Utilizes safe transfer techniques and ambulation.
Assists with normal range of motion and body positioning.
Provides patient with adequate nutrition and fluid intake.
Performs simple procedures as an extension of therapy or nursing services.
Assists in administering medications that are ordinarily self-administered.
Observes, reports and documents changes in patient's medical, nursing, rehabilitative and social needs or expectations of service promptly to NHS Sequoyah Home Health supervisor or professional.
During supervisory visits, demonstrates utilization of good basic infection control procedures and following of NHS Sequoyah Home Health procedures, especially with regard to infection control, handling of hazardous wastes, and safety measures in all instances.
Accurately documents care provided and submits required forms within 24 hours of visit.
Respects rights of patient including the patient's right to privacy and respect for the patient's property; adheres to NHS Sequoyah Home Health Name confidentiality policy in all instances and maintains professional objectivity.
All services rendered are provided in strict accordance with the written plan of care.
Maintains NHS Sequoyah Home Health property, supplies and equipment in a manner that demonstrates ownership
and accountability.
Able to develop cooperation and collaborative work efforts that generally benefit all involved parties.
Demonstrates the initiative to meet the needs of the NHS Sequoyah Home Health by assisting coworkers whenever work load permits.
Demonstrates the ability to work in a team responsibly but independently, without direct supervision.
Demonstrates the knowledge and skills necessary to provide care to and communicate with a primarily geriatric population.
Demonstrate knowledges of the principles of growth and development over the life span as related to job duties.
Assesses data reflective of a patient's status and report the appropriate information relative to the patient's age-specific needs.
Follows policies and procedures regarding attendance and work hours.
Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops.
Attends meetings as required.
Performs other related duties as assigned or requested.
Minimum Education- Successful completion of a state-established or other training program that
meets the federal requirements. Successful completion of a competency evaluation program or state
licensure program that meets the federal requirements; or successful completion of a nurse aide
training approved by the state as meeting the federal requirements and is currently listed in good
standing on the state nurse aide registry; and has performed services for compensation with the last
consecutive 24 months.
Required Licensure/Certification - Current driver's license; Proof of automobile insurance; CPR/BLS
