Job summary
The Health Access Network (HAN) at OSU-Center of Health Sciences is recruiting for a Care Manager position in Ada, OK.The Care Manager offers case management/care coordination to persons with complex health care needs.The position contributes to improving access, quality, and continuity of care to SoonerCare members.

Responsibilities
• Offer case management/care coordination to persons with complex health care needs.• Facilitate members' access to all levels of care utilizing case management/care coordination services.• Provide telemedicine services, electronic health records, improved access to specialty care services, and expanded quality improvement strategies.• Manage environmental hazards, including exposure to hazardous materials and blood borne pathogens.
Requirements
• Associate's degree in Nursing plus 2 years nursing experience OR, Bachelor's degree in Nursing plus 1 year of experience as an RN OR, Master's degree in Nursing OR, Master degree in Social Work plus 1 year of experience as an LCSW.• Current & active RN, or LCSW licensure issued by Oklahoma State Boards of Nursing or Social Work.• Valid Driver License.• Case management/outpatient clinic experience.• Skills and knowledge in chronic disease management.• Exceptional interpersonal communication skills - to include writing/documentation skills.• Critical thinking skills and sound judgment.• Initiative and ability to work independently and within a team.• Ability to meet deadlines and prioritize workload.• Exhibit cultural sensitivity and the ability to work with diverse patient population and professional staff, faculty, and medical students within academic health care setting.• Computer skills.• Preferred: A minimum of 3 years clinical practice experience. Case management certification (or eligible to obtain).
