Diabetes Management - Haitian  Creole Speaking NP/ PA  Job Location: Princeton, NJ (Onsite +Telemedicine).
Job Type: Per Diem, 1099 contract.
Pay Rate: $130/hr for onsite patient care, $100/hr for telemedicine 
Schedule: Starting in April 2024, 1-2 days/month onsite, 5-10 hours/week telemedicine, flexible.
Full Description: VitalCheck Wellness is seeking a Nurse Practitioner or Physician Assistant to provide diabetes management programs to some of our clients in Princeton, NJ area. The NP/ PA will work directly with pre-diabetic or early-stage diabetes patients on a multi-week program to help improve their wellness and lifestyle. Our office arranges the patient appointments and does the administrative work, giving you the time and luxury to provide clinical one-on-one patient care.
Responsibilities: Provide general wellness checkups. Consult with patients to understand their health concerns. Advise/refer patients for follow-up care.
Provide diabetes management program to patients based on our program guideline. Consult with patients to understand their health concerns especially around diabetes. Educate patients around healthy eating and lifestyle. Educate patients around glucose monitoring. Requirements: Family Nurse Practitioner (FNP)/ (FNP-C)/ (FNP-BC)/ Acute Care Nurse Practitioner (ACNP) certified.  Background in internal medicine, family practice, urgent care, or emergency room. Minimum 3-5 years of work experience following post graduate training. Must have current NJ license to practice in the state of NJ.  Must be fluent in Creole. Experience with diabetes management.
Preventive medicine oriented. Patient education driven. Must be proficient with computers, customer service-oriented, and able to work independently. Must be able to commute to Princeton, NJ.
