Kids Above All is seeking a Case Manager for our Naperville, IL location.
Job Summary: The Case Manager position will coordinate and/or provide services and activities for youth in a group home setting. This includes but is not limited to medical, school and employment appointments and family visitation. This position will also act as a liaison between the Agency and other agencies associated with the care of the clients (i.e., DCFS, court, etc.). The Case Manager will also collaborate with staff and other key individuals toward implementing a permanency plan.
Essential Functions:
Carry a client caseload with an emphasis on meeting the environmental and psychosocial needs of the clientProvide casework and counseling services to youth and their parents/guardians regarding permanency planArrange and/or supervise visits with parents/guardians and familiesComplete data entry of client information into client management systemConsult and review cases with Supervisor and other team membersPerform a number of case management duties including preparing for and attending court hearings, administrative case reviews, preparing reports and providing testimony as requiredStay current in job and/or client specific competenciesActively participate in Continuous Quality Improvement initiatives and activitiesOther duties as assigned to fulfill the mission of the Department and the AgencyParticipate in team meetings, case reviews, staffing, etc.Collaboratively evaluates and plans for treatment of child where problems existCooperates with medical staff during examination for consultation and staffing of childrenIdentifies and understands the scope of community resources and how they can meet the needs of children and families served Qualifications:
Bachelor's degree in Human Services, or related field. One to three years of experience requiredPassing of all DCFS required exams and certifications, including DCFS Child Welfare LicensureDriver approval standards, plus insured automobile that meets standards for safety and drive-abilityMust have a perceptive awareness of the needs and responses of children, and capacity to work well with adultsAbility to organize work and prepare required reports.Flexibility and willingness to work evening and/or weekend hours, if needed to carry out all job responsibilitiesBasic computer skills and knowledge of Microsoft Word requiredSalary Range: $47,400-$50,900
