Geriatric Case Manager

Responsibilities:
Conduct client assessments with care and empathy, ensuring their needs are understood and addressed
Serve as a dedicated advocate for our clients, liaising with facilities, physicians, and families to ensure the best possible care
Manage appointments, coordinate meetings, and develop personalized care plans to support our clients' well-being
Collaborate closely with our trusted network of community partners to provide comprehensive care solutions
This role offers flexible hours and the autonomy to set your own schedule, with the opportunity to work 30-35 hours per week. We understand the importance of work-life balance, so occasional weekend or evening commitments are reimbursed at a competitive rate.
Qualifications:
Strong interpersonal skills and a compassionate approach to caregiving
Excellent communication abilities, both verbal and written
Exceptional organizational and problem-solving capabilities
Dedication to maintaining accurate records and meeting objectives
Bachelor's degree preferred, along with one to three years of relevant experience
Experience in geriatric care management, healthcare, medical social work, or case management is a plus
Must have access to a computer for maintaining electronic records and a personal vehicle for transportation
Current residency in Northern Virginia, specifically eastern Fairfax, Arlington, or Alexandria counties
