The Recovery Case Manager(RCM) will meet with their clients regularly on a one-on-one basis. The RCM is the primary contact for a client's family, loved ones, and referral sources. They are responsible for the planning and implementation of client's aftercare. RCM's also serve to assist clients with their outside needs (e.g. court hearings, application for unemployment benefits, FMLA).
Responsibilities: Help hold clients accountable for their actions in the community and the completion of their workFacilitate and provide timely documentationMonitor compliance on client contracts, phone policy, "buddy" relations. Ensure that all pertinent information is notated on RSS office board at shift changeAbility to work in a fast-paced, dynamic environmentAbility to communicate effectively in written, electronic, and oral formAbility to make sound decisions and/or find alternative solutions Make weekly calls to the client's family (more communication may be required). This communication will be logged in the EMR. Phone calls will address client progress, behavior, contracts, growth opportunities, programming, and aftercare. RCM must be prepared to discuss enabling, codependency, and/or Al-Anon. RCM is responsible for scheduling all calls and face-to-face meetings with family members and/or loved onesFamily calls are to be within the first 24 hours of admissionReferral calls are to be made within the first 24 hours of admission.
