Field Nurse Practitioner – Arizona Candidate must reside And be licensed in the state of Arizona.**
Location: This Field Based position will be visiting patients/members in assisted living facilities and other facilities in Phoenix, Glendale, Peoria and Sun City, Arizona. Eligible for mileage reimbursement. * Required Work Shift: 5 days a week, Monday thru Friday, 40 hours a week. The Field Nurse Practitioner is responsible for managing patient care and treatment (in person, telephonically and/or video) in collaboration with the physician.
How will you make an impact: + Conduct daily visits with members/patients that reside in either Group Homes, Assisted Living Facilities, and/or Long Term Care Facilities.
+ Perform health assessments and preventative health measures within prescribed guidelines.
+ Order, interpret and evaluate diagnostic tests to identify and assess patient's clinical problems and health care needs.
+ Discuss case with physician.
+ Formulate and document care plan.
+ Prescribe medication or other forms of treatment.
+ Participate in a rotating On-Call program, which may include after-hours, weekend and holiday calls and visitations.
Minimum Requirements: + Requires an MS in Nursing and minimum of 1 year of Practicing nurse practitioner experience in either primary care, family practice, internal medicine or geriatrics ; or any combination of education and experience, which would provide an equivalent background.
+ Current, unrestricted RN license And NP license in the state of Arizona is required .
+ Possession of DEA registration or eligibility is required.
+ For Carelon Health business units, satisfactory completion of a Tuberculosis test is a requirement for this position.
Preferred Skills, Capabilities and Experiences: + Experience working with the adult and geriatric patient populations.
+ Board Certified w/ AANP or ANCC .
+ Experience working in home health, palliative and/or hospice care.
