Job Type: Full-timeSalary: $52,000.00 per yearWe are looking for an individual to join our Care Team. This position is primarily responsible for balancing relationships with County Service Coordinators, our clients, Families, and our DSPs.Responsibilities:Family Service representatives (FSR) report to the Director of the Care Team. As a FSR you will oversee a caseload of clients. You will be primarily responsible for completing regular check-ins and directing their support needs. Responsibilities include but are not limited to:Organize Client SupportsManage Crisis SituationsChecking/responding to emails/calls/texts dailyCompleting client intakesRegularly visiting clients/families within their homesOngoing communication with clients/families/Service CoordinatorOngoing support to families/Client/DSPsThis is a growing position; we are a dynamic company that stays on our toes. We are looking for flexible, charismatic, and "can-do" candidates to grow and change with us.
