Diabetes Educator Details Essential Job Duties:
Performs comprehensive learning needs assessment with regard to physical, psychological, and environmental factors that may affect the plan of care. Plan of care which includes goals and prioritized nursing/medical approaches derived from data collected. Utilizes additional resources in goal setting and meeting learning needs. Implements strategies to provide for patient's participation in goal setting, health maintenance and maximization of health capabilities. Notifies physician of significant changes in patient's condition and potential or suspected complications. Evaluates the patient for diabetes knowledge and status of diabetes management. Qualifications & Licenses:
Currently licensed by the Indiana State Board of Nurses' Registration and Nursing Education as a registered nurse with a minimum of one year in diabetes education. Certification by the American Association of Diabetes Educators (CDE) required or willing to obtain within 2 years of hire. Possible extension may be granted of up to 6 months to accommodate testing availability depending on hire
