Responsibilities
• Keep abreast of current regulations and make recommendations• Implement policies and procedures for identifying resident needs• Participate in community planning for facility interests• Participate in discharge planning and resident assessments• Provide consultation to staff and community agencies• Review and develop plan of correction for deficiencies• Maintain reference library and standards of practice• Conduct and document social services evaluation• Maintain communications with caseworkers• Provide social work consultation to residents, families, and staff
Requirements
• Bachelor's Degree and LSW or MSW from approved school of Social Work• Minimum of one year experience• Ability to assess resident needs and develop plan of care• Knowledge of accreditation standards and compliance requirements• Member in good standing in National Association of Social Workers• Qualified, compassionate, and dedicated
