POSITION SUMMARY: The Coordinated Entry Prevention Navigator works closely with the Coordinated Entry Team, community partners, and internal IMPACT departments to provide telephonic and in-person assessment, and resource development to individuals and families who are experiencing a housing crisis. The goal of this intervention is to divert or prevent people from becoming literally homeless, connect them to community and Coordinated Entry resources when needed, and to work with the Coordinated Entry System to facilitate connection to emergency and permanent housing solutions when needed. The Coordinated Entry Prevention Navigator will provide quality professional services to internal and external customers and implement IMPACT's philosophy of teamwork, customer service, and value added, in the call center and in the community.
PRINCIPAL RESPONSIBILITIES: Respond telephonically to referrals from community partners including 211, street outreach teams, school systems, child welfare, and others. At least weekly and when needed, complete assessments in person at specific community sites. Provide assessment of current housing circumstances and needs, problem solving, information and referral, and crisis intervention, when necessary, in an empathetic and nonjudgmental manner. Assist clients in developing an action plan for preventing homelessness within the scope of eligible family, health, and social services that appropriately meets their needs. Accurately document client contacts through client profiles and case notes in a computerized database and other means. Provide advocacy for clients experiencing difficulties or lacking in abilities needed to make effective contacts with agencies, programs or groups. Participate in cross training in order to fill in for and assist other Coordinated Entry team members as needed. Participate in ongoing IMPACT training and staff meetings. Participate in ongoing Continuum of Care meetings as appropriate. Keep informed of and demonstrate knowledge of all special service programs. Ensure that IMPACT is compliant with applicable federal, state, and local laws ensuring client confidentiality. Assure that data entered into the Wisconsin HMIS database is accurate, up-to-date and entered according to the standards and practices required by the Milwaukee County Continuum of Care and all governing bodies. Participate in training related to homelessness, HMIS and any other training associated with shelter systems and housing and others as needed. Build and sustain strong working relationships with Milwaukee County emergency shelters, Milwaukee County Continuum of Care members and other providers of services to the homeless and those at risk of homelessness, as needed. Scope of Providing Services to IMPACT Clients:
-Participate in a comprehensive initial training program.
-Provide assessment, problem solving, information and referral, and crisis intervention when necessary in an empathetic and nonjudgmental manner.
-Assists clients in developing and executing an action plan.
- Provide advocacy for customers experiencing difficulties or lacking in abilities needed to make effective contacts with agencies, programs or groups.
- Participate in ongoing training and staff meetings, including Leadership Team meetings.
- Keep informed of and demonstrates knowledge of all special service programs. Standard IMPACT Requirements:
- Responsible for protecting client confidentiality and adhering to all state and federal laws and regulations regarding the protection to AODA and mental health information.
-Protects against unauthorized access, modifications, destruction, and disclosure as defined by IMPACT and relevant federal, state, and local laws. Actively participate in and support the organization's Diversity, Equity, and Inclusion (DEI) initiatives. Participate in cross training and cover any and all duties as assigned of the CE team, outside of this specific job description. Meet all funding requirements to include trainings and other annual requirements. ABILITIES/SKILLS: Possesses cultural competency skills to work effectively with diverse customers and staff. Demonstrates strong assessment, empathy, and client advocacy skills. Ability to respond effectively to a variety of callers' social service needs. Exceptional customer service skills, both internal and external. Demonstrated strong interpersonal skills to build positive working relationships with outside agencies and organizations. Exceptional verbal and written communication skills. Good judgment, problem solving and decision-making skills. Self-motivated. Demonstrated organization and time management skills. Ability to work effectively in a team-oriented environment. Ability to adapt to and apply on-going program development changes. Bilingual ability preferred. Computer proficiency necessary. QUALIFICATIONS & WORK SCHEDULE: Bachelor's degree in Social Work or human service studies preferred but a strong background of applicable work experience may substitute Experience working with people experiencing homelessness strongly preferred Computer software expertise: Word, Excel; HMIS (Clarity) experience a plus; Ability to learn new software systems Valid WI Driver's License required with a current auto insurance policy and reliable vehicle. Reside in the State of Wisconsin and within 50 miles of IMPACT
The position is generally hybrid, Monday - Friday from 12pm - 8pm. However, for training purposes, candidate must be able to attend training onsite, Monday-Friday from 8:30am-4:30pm for the first 90 days of employment. Onsite hours may be located at the IMPACT office or at a CE Assessment Site. The position will work, as necessary to achieve success, to coordinate work, consult with colleagues, attend meetings, and participate in agency activities and events.
