This position focuses on assisting older adults in the PACE program obtain needed resources which will enable them to remain in their own homes. We're looking for a Community Resource Specialist to join our PACE team in Kansas City, KS! We are looking for someone with Bachelor's degree in social work, human services or other relevant program. You may be just what we're looking for!
Midland Care's Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.
For Midland Care PACE participants, PACE means a team of experienced doctors, nurses and other health care professionals getting to know the participant and working with him or her to design a plan to aid in achieving health goals.
The purpose of this position is to provide services to meet the social and/or emotional needs that affect the clients' ability to achieve their goals; participate in the development of patent/client comprehensive care plans; develop policies to provide social services to clients in compliance with federal, state and local regulations. This position is supervised as designated on the agency organizational chart.
Employment Status: Full-Time|Monday – Friday, 8:00 a.m. to 5:00 p.m.
Pay range beginning at $20.86 - pay based on experience Qualifications Required: Bachelors degree in Social Work. Must have knowledge and skills with regard to health care coordination, pain/symptom management, crisis intervention and counseling. Must possess valid Kansas Driver's License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by Midland policy.
