The Community Health Worker and Engagement Specialist builds visibility and credibility of organization's products and services throughout community via a variety of interactions.
Strategizes methods to scale communities and drive greater connections among community members. Engages new community members.
Understands own work area professional concepts/standards, regulations, strategies and operating standards.
Makes decisions regarding own work approach/priorities and follows direction.
Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation
Essential Role Functions and Responsibilities: Conduct in-person non-clinical assessment(s) to understand enrollee care needs, preferences, and socioeconomic barriers and evaluate the home environment as needed.
Work collaboratively with other Humana associates as a member of Humana's Care Management Support teams, including case managers, housing specialists, SDOH coordinators, and Community Education Representatives (CER) and serve as a member of enrollee multi-disciplinary care teams (MDT), as requested.
Assess enrollee barriers to healthy living and accessing healthcare services and arrange needed social services and supports.
Act as an enrollee advocate with providers, community resources, schools, and others, including accompanying enrollees to provider office visits as requested.
Monitor enrollee compliance with their care plan and provide motivational interviewing to support medication and treatment adherence.
Provide social support to help boost enrollees' morale and sense of self-worth, serving as a trustworthy, reliable, non-judgmental, consistent, and accepting member of the enrollee's MDT.
Support enrollee self-management through the provision of culturally appropriate health education and health coaching.
Serve as a key knowledge source for community services and information for enrollees and other Humana associates.
Attend community events to connect with current and potential enrollees and provide education on Humana Healthy Horizons in Kentucky.
Conduct research and in-person outreach to locate difficult-to-contact enrollees to increase assessment completion and participation in clinical programs.
Build and maintain relationships with providers and community resources to support enrollee identification and referrals.
Implement community assessments to identify community resource gaps.
Regular travel to conduct member visits, provider visits and community-based visits as needed to ensure effective administration of the program
Perform any other job-related instructions, as requested

Use your skills to make an impact Required Qualifications Minimum two (2) years professional human services experience OR recent service in the same role, working in-person with consumers conducting assessments and/or developing care plans.
Minimum (1) one year of community outreach and/or marketing and sales experience
Community Health Worker Certification must be obtained within 6 months of hire date.
Excellent written and verbal communication skills
Proficient in Microsoft Office (Excel, Word, PPT).
Experience using a variety of electronic information applications/software programs.
Preferred Qualifications Bachelor's Degree in Social Work or Health related services
Bi-lingual: Spanish
Previous experience working in the Community
Home Health experience
Knowledge of community health and social service agencies and additional community resources.
Additional Information Workstyle: Combination Remote Work at Home and Field based which includes performing core duties at one or more non-Humana locations.
Location: must reside within 50-mile radius of either Prestonsburg or another population dense community in Eastern KY (Floyd County)
Travel: 60-80% within 50-mile radius of 50-mile radius of either Prestonsburg or another population dense community in Eastern KY as business needs require
Schedule: Must be available to work Monday - Friday between the hours of 8am-6pm Eastern Time Zone (EST) but be flexible with your hours based on business needs. Community events often happen on evenings or weekends.
Work at Home Guidance To ensure Home or Hybrid Home/Office employees' ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:
At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested
Satellite, cellular and microwave connection can be used only if approved by leadership
Employees who live and work from Home in the state of California, Illinois, Montana, or South Dakota will be provided a bi-weekly payment for their internet expense.
Humana will provide Home or Hybrid Home/Office employees with telephone equipment appropriate to meet the business requirements for their position/job.
Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
Tuberculosis (TB) screening This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
Driver Safety This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license, reliable transportation, and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
Language Proficiency Any Humana employee who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
