Clinical Pharmacy Intern

Join our team as a Clinical Pharmacy Intern and gain hands-on experience in a dynamic healthcare environment. Duties include: Responsibilities Conduct medication reconciliation as needed as part of patient care plans. Make outbound enrollment calls to patients and assist the Patient Enrollment Specialists. Prepare clinical information to support pharmacist-patient engagements. Coordinate care with doctor's offices, pharmacists, and other healthcare professionals. Engage patients remotely, via video or audio, to gather medication and health information. Learn about current guidelines and collaborative treatment decision-making processes. Gain insights into data analysis for evaluating clinical interventions. Explore the development of healthcare software systems. Independently complete projects under pharmacist guidance. Support the clinical pharmacy team's operations and complete assigned research projects. Participate in the software development process to enhance healthcare systems. Identify areas for improvement and effectively communicate recommendations. Ensure compliance with regulations regarding patient health information handling. Perform other duties as assigned.
