The Multnomah County Health Center is seeking an enthusiastic and dedicated Clinical Pharmacist to join our primary care teams. As a Clinical Pharmacist, you'll play a pivotal role in providing comprehensive clinical pharmacy services to both our staff and clients.
Clinical Pharmacists work as members of multidisciplinary primary care teams to assure that patients receive quality pharmaceutical care delivered in a safe and efficient manner in Multnomah County's Community Health Center.
As the Clinical Pharmacist, you will:
Collaborate with primary care teams to optimize medication therapy management.
Provide direct patient care to adult clients of the health center including, but not limited to; Comprehensive Medication Reviews, Medication Therapy Management, Collaborative Drug Therapy Management, Transitions of Care Medication Reconciliation, and education related to the safe and appropriate use of medications.
Provide pharmacotherapy education and consultation to medical and pharmacy staff.
Facilitate positive and constructive interactions with clinical staff, clients, and the pharmacy team.
Participate in interdisciplinary committees, patient care teams, drug protocol reviews and medication use evaluations.
Precept ambulatory care pharmacy APPE students.

page. Minimum Qualifications/Transferable Skills*:
Pharm.D. or Bachelor of Pharmacy with equivalent experience from an accredited college or university.
Completion of PGY-1 or PGY-2 Pharmacy Residency.
Strong clinical pharmacy skills and knowledge.
Ability to work effectively in a multidisciplinary team environment.
Commitment to promoting diversity, equity, and inclusion.
Required License(s)/Certification(s):
Licensed as a current registered pharmacist in the state of Oregon by the Oregon Board of Pharmacy.
Valid CPR / BLS Certification upon hire.
Preferred Qualifications/ Transferable Skills*:
Comprehensive clinical pharmacy knowledge and practice experience in a primary care setting, AND
Able to independently evaluate and synthesize data and apply evidence-based medicine principles to practice, AND
Demonstrated ability to effectively establish and maintain working relationships with peers and constituents at all levels of the organization, AND
Excellent interpersonal communication skills and the ability to communicate effectively in verbal and written form with the health care team and with patients.
