Fulfill your purpose at CLM: Center for Life Management (CLM) provides Southern New Hampshire's most comprehensive array of outpatient mental health services for adults and elders. Our multi-disciplined treatment teams work with individuals ages 18+ who experience severe and persistent mental illness and co-occurring disorders.
Assertive Community Treatment (ACT) is an evidenced based practice model that provides an intensive and highly integrated approach to community mental health services. ACT support helps people with severe and persistent mental illness become independent and live in the community as they work toward their recovery goals. ACT support also helps with reducing homelessness and unnecessary hospital stays. The ACT Clinical Case Manager provides outpatient clinical services including diagnostic assessment, treatment planning, crisis management, therapy, care planning, resource identification, benefit coordination, service monitoring and community-based support. This is a full time, 40 hour per week position.
Major Functional Areas and Description of Duties
Provide individual diagnostic assessment and treatment planning specifying presenting problem, objectives, interventions, and desired outcomes; Assist clients with applying for and maintaining adequate medical coverage to support consistent access to whole health care and treatment; Provide outreach, advocacy, coordination, resource identification, linkage and monitoring of internal and external supports and services; Provide crisis management, therapy and community-based support in a variety of settings including the home, public, or office to support client's treatment objectives; Work collaboratively with other ACT Team members, natural supports, and external providers to ensure wrap-around services are in alignment with the ACT model; Provide on call, after hours, holiday and weekend coverage on a rotating basis; Provide set case management contacts per month and set hours of therapy and/or community-based functional support services per week (eligible for incentive pay upon exceeding productivity expectation); Serve on appropriate committees, both internal and external, as determined by supervisor; Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients; Execute all required services and timely documentation in accordance with agency policies and federal/state standards; Maintain professional memberships, licenses, accreditation, certifications, etc.; Represent CLM in a professional and appropriate manner in all settings; Perform all other duties as assigned by supervisor. Qualifications and Skills
Master's degree required (social work, clinical mental health counseling, psychology, etc.), previous experience working with co-occurring disorders preferred; Ability to exercise sound clinical judgment in routine, urgent, and emergent clinical situations; Proficient computer skills, specifically with all Microsoft office applications; Ability to document effectively and efficiently in course of clinical work; Ability to navigate an electronic health record; Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision; Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services; Effective communication skills, organizational skills, and ability to multi-task. Physical Demands and Working Conditions
A majority of the work will be visiting clients in the community and also in their homes; occasional office and remote work permitted based on client needs. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.
