Responsibilities
• Provide clinical expertise in diabetes management• Educate and create treatment plans for diabetic patients• Develop programming to achieve positive change in diabetic patient population• Follow ADA guidelines and educate newly diagnosed diabetic patients• Collaborate with providers and health coaches• Work with poorly controlled diabetic patients to improve disease management• Promote self-management to optimize health outcomes• Organize group programming and attend outreach activities• Instruct patients and family members on diabetes management• Assist in the development of diabetes and health education guidelines
Requirements
• 2+ years of direct patient care experience treating a diabetic population• Certified Diabetes Educator license• Registered Dietician and/or Health Coach Certification preferred• Knowledge and experience in health risk assessments, lifestyle risk reduction, disease management, and health promotion• Certified in BLS• Commitment to Ambassadors Code values
