Responsibilities
• Provide screening, intake, and case management for program participants• Conduct triage assessments for the Coordinated Entry for All system• Assist with completing paperwork for community supports• Attend housing appointments with clients• Work on goals related to obtaining housing and increasing incomes• Develop plans for stability in various life domains• Establish connections and provide referrals to other social service agencies• Assess housing options and basic needs• Provide follow-up services to program participants working toward employment goals• Complete assessments to identify potential resources
Requirements
• High School Diploma or GED equivalent• 2 years direct experience with homeless and/or at-risk populations• At least 1 year of case management or equivalent experience• Knowledge of social services resources and programs in King County• Ability to support shelter rules and work with culturally diverse clients• Computer skills and ability to learn and use CCS HMIS• Pass required background clearances
