Job summary
The Housing Focused Case Manager provides trauma-informed case management services to individuals experiencing homelessness.Their goal is to help these individuals achieve stable housing solutions quickly and efficiently.They work with the shelter team to identify common barriers faced by individuals in shelter and establish strategies to address their needs.

Responsibilities
• Help individuals experiencing homelessness get into housing.• Assist guests in identifying barriers and connecting them with community resources.• Celebrate and assist guests in moving into their new homes.• Use agency vehicles to provide transportation for guests in pursuit of housing.• Manage a caseload and assist diverse guests in establishing goals and connecting them with resources and housing options.• Maintain healthy boundaries with guests and collaborate with all departments to promote a healthy team environment.• Engage and network with community partners to find housing options and additional resources.• Attend team meetings and contribute to case conferencing conversations.• Keep accurate records, case management notes, and statistical data as required.• Provide direct, trauma-informed case management services to guests, including crisis intervention, assessments, goal setting, and problem-solving.• Assist with de-escalation and crisis intervention with guests in crisis.• Collaborate with crisis clinicians and others to support individuals with mental and behavioral health barriers.• Demonstrate awareness and sensitivity to diverse populations.• Promote housing first, harm reduction, and low barrier access to resources.
Requirements
• Experience working with diverse and vulnerable populations is preferred.• Lived, worked, or volunteered experience is preferred, or a degree in a related field.• Ability to move 25 pounds.• Ability to stand and walk for at least an hour at a time.• Ability and license to drive a vehicle.
