Description: We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for coordinating and managing the care of clients in your caseload, ensuring that they receive the support and resources they need to achieve their goals. The ideal candidate will have excellent communication and interpersonal skills, as well as strong organizational and problem-solving abilities. As a Case Manager you are expected to Provide high quality social work case management services to English and Spanish speaking victims of crime guided by agency policies and programs. The position utilizes the Strengths model of case management, and requires proficiency in Spanish and cultural-competence.Responsibilities:
Provides case management services to clients in Missouri. As funding is available, provide case management services to other clients receiving counseling at TFC through grant fundingSupports individuals and families by addressing barriers to the client's overall health and success which could include: transportation, connecting to community resources, advocacy with legal or medical resources, application for Crime Victim's Compensation, interpreting/translation, or assistance with filing orders of protectionDistributes materials, makes contacts within the community, and participates in community meetings, as assigned; to develop awareness of agency services and increase service numbers by engaging consumers and potential consumersContributes to participant success by providing information and referral services to consumers seeking assistance outside of the scope of services provided by The Family Conservancy, as requestedDevelop and execute client specific service plans in concert with clientsTimely documentation of services providedWorks with interpreters to provide quality service provision to individuals that neither speak English or SpanishParticipates in on-going program development, performing assigned tasks to either create or maintain program services including participating in utilization review and other meetings as appropriateFunctions as a participating member of the program team, maintaining effective relationships both with team members and community partnersAs a back up to the Bilingual Scheduling Specialist, participates in intake service (phone and walk-ins) and provides referrals as needed, making effective use of agency and community resourcesEffectively utilize agency technology to accomplish tasks. (Microsoft Office Suite; Google Mail, Calendar & Drive; program specific software; printers/scanners/fax/phone)Participates in weekly supervision meetings including problem solving with supervisor and other team members to support effective client servicesDemonstrates Bilingual fluency in written and oral Spanish/English which permits linguistic, cultural interpretation services and advocacy for consumersAdheres to Agency Vehicle Procedures while providing transportation for clients when needed and within the service descriptors of the programComplies with Grantor's reporting requirements by providing information for monthly program reports and service analysisAbides by all agency policies, state and federal laws concerning mandated reporting by notifying state protective services of intake calls or active case management client cases related to suspected child and/or elder abuse and neglectAttends trainings relevant to Trauma Informed Care practices, learns about the Trauma InformedPractice Approach Principles and utilizes them with clients and staffOther duties as assigned by clinical director or executive leadershipRequirements

Bachelor's degree in social work or related field requiredBilingual fluency in written and spoken English/Spanish requiredPossess ability to work with families in a supportive manner throughout assessment and referral processesDemonstrate a strong work ethic, sound judgement, diplomacy and tact.Collaborate with a diverse group of individuals, staff and community partners; serve as a positive role model and team player.Travel throughout the facility, metropolitan area and counties within the service delivery area is requiredAble to serve clients from diverse cultural and socio-economic backgrounds in their home or community setting.LMSW or LBSW license preferredBilingual fluency in written and spoken English/Spanish preferredPrior relevant work experience with socioeconomically disadvantaged populations in a community or other relevant settingsWorking knowledge of community resourcesValid driver's license and willingness to transport clients in personal vehicleIf you are passionate about helping others and are committed to providing high-quality care, we encourage you to apply for this rewarding position.
