Education: Graduate of a RN Program Bachelor's Degree is preferred Schedule: Monday - Friday 8:00am-4:30pm Work Remotely: No Position Summary: Case Manager will be responsible for assigning, monitoring, and organizing cases for Social Work.
Essential Duties and Responsibilities: Assessing clients' physical and mental wellness, needs, preferences, and abilities and using these to develop tailored care plans. Working with clients, family and friend support networks and healthcare professionals to put care plans in place. Listening to clients' concerns and providing counseling or intervention as required. Recording clients' progress including charting referrals, home visits and other notable interactions Evaluating clients' progress periodically and making adjustments as needed to improve outcomes Following up with discharged clients to ensure they were satisfied with services and they are still in good physical and mental health. Report abnormal changes or patterns in nursing staff. Completed documentation of required forms/documents. Subject to irregular hours. May be required to wear personal protective equipment as directed. Follow infection control and isolations guidelines. Adhere to all hospital polices and procedures. Perform other duties as assigned. License/Certification: Current Mississippi RN License is required. BLS is required. Skills: Excellent verbal and written communication skills.
