Experience:
• 12 months of Case Management experience is required.
• 3 months of experience with McKesson is preferred.
Requirements:
• Candidates must have a Vermont license or compact license in hand at the time of application.

Additional Details:
Guaranteed Hours: 36
Shift Information: Work schedule consists of Monday through Friday 8-5, with a rotation to include Saturdays approximately every fourth week, allowing for a day off during the week to facilitate the rotation.
Experience with hospice case management required
Experience providing hospice care in a home setting required
Experience with Netsmart EMR preferred
Looking for a nurse with NO time off requests for this contract
