Fulfill your purpose at CLM: Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental health services for adults and elders. Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness and/or co-occurring disorders. CLM offers many evidence-based and best practices to deliver effective care coordination and case management services including Critical Time Intervention (CTI). CTI is a time-limited case management model designed to prevent homelessness in people with mental illness following discharge from hospitals, shelters, prisons and other institutions. CTI mobilizes support for society's most vulnerable individuals during periods of transition. It facilitates community integration and continuity of care by ensuring that a person has enduring ties to their community and support systems during these critical periods. The Critical Time Intervention (CTI) Coach provides treatment to those involved in the CTI program with the primary focus on case management, linking individuals to long term clinical and social supports. This is a full time, 40 hour per week position.
Major Functional Areas and Description of Duties
Following the evidence-based practice of Critical Time Intervention (CTI), engage clients in the following stages: Pre-CTI: Rapidly engage referred individuals to facilitate enrollment; Meet with potential individual pre-discharge, obtaining agreement/consent, and engaging with appropriate collateral contacts at the inpatient setting; Phase 1: Accompany individuals on the day of discharge to support and assess primary needs and barriers; Ensure connection to primary supports, including but not limited to mental health services, Social Determinates of Health (SDoH) supports as needed, and social/support groups; Strengthen supports through active engagement and intervention, both with the individual and with collateral contacts by: Making home visits as needed; Engaging in collaborative assessments; Meeting with existing supports; Introducing client to new supports; Giving support and advice to client and caregivers. Phase 2: Continue to strengthen supports to community providers through direct and collateral contacts; Engage with and assisting primary supports to promote attendance at treatment and SDoH appointments, with progressively less of a direct support role as longer term services take hold by: Observing operation of support network; Mediating conflicts between client and caregivers; Helping modify network as necessary; Encouraging client to take on more responsibility. Phase 3: Focus on long term stability for the individual, ensuring that all long-term services remain in place and that there are no impending, avoidable disruptions by: Observing operation of support network; Mediating conflicts between client and caregivers; Helping modify network as necessary; Encouraging client to take on more responsibility. Engage in-person interventions and other interventions to best support client needs; Work collaboratively with other treatment team members, natural supports, and other external providers to ensure wrap-around care is provided to the client; Provide timely documentation on each service delivered; Engage in appropriate consultation meetings and committees, both internally and externally, as determined by supervisor; Function professionally and in a manner that protects the integrity, confidentiality and rights of all clients; Execute all required services and documentation in accordance with agency policies and federal/state standards; Maintain professional memberships, licenses, accreditation, certifications, etc.; Adhere to CLM's Code of Conduct and professional ethics; Represent CLM in a professional and appropriate manner in all settings; Perform all other duties as assigned by supervisor. Qualifications and Skills
Associate degree in a human service related field (social work, psychology) or related field and equivalent experience. Bachelor's degree preferred; Experience with behavioral health or social determinants of health (SDoH) systems, including case management or delivery of clinical intervention; Experience with social determinants of health-related programs, including but not limited to housing, employment, food insecurity, and entitlements; Understanding of NH systems of care, with experience working between systems; Knowledge of treatment modalities and interventions such as service coordination, behavior management, independent living skills training, supportive counseling, family education, discharge planning and crisis intervention; Ability to exercise sound clinical judgment in routine, urgent, and emergent situations; Physically and mentally capable of performing multiple tasks under stressful situations; Proficient computer skills and knowledge of all Microsoft office applications (Word, Excel, Publisher, Power Point, Outlook, TEAMS, etc.); Ability to document effectively and efficiently in course of clinical work; Ability to navigate an electronic health record; Advanced documentation skills and familiarity with data reports/spreadsheets; Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision; Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services; Demonstrate effective communication skills, organizational skills, and ability to multi-task. Physical Demands and Working Conditions
The majority of the work will be visiting clients in the community and also in their homes. Manual dexterity is needed to use computers and standard office equipment. The ability to stand and sit for extended periods of time is required. Reliable internet access is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.
