The team care manager for the Center of Excellence (COE) is responsible for conducting facility and community based comprehensive biological, psychological, social, level-of-care, and spiritual assessments; assessing and motivating the opioid population to address recovery needs impacted by areas of life domains; making appropriate referrals to community and treatment supports; and coordinate level of care transition needs. In addition, provides resources and education to both persons served and supports of persons served. JOB DUTIES AND RESPONSIBILITIES: Complete mobile assessments as requested that may be located at emergency rooms, medical offices, and the criminal justice system. Coordinate transportation needs to treatment facilities. Ensure warm handoffs with COE team and the St. Luke's Penn Foundation service delivery. Complete referrals and follow-up to all behavioral supports identified through assessment process Identify COE persons served that have disconnected from recommended clinical care through electronic health record review with a minimum of a weekly review. PHYSICAL AND SENSORY REQUIREMENTS: Must have ability to sit for 2 total hours per day, and up to 1 total consecutive hour. Must be able to stand for 8 total hours per day and up to 2 total consecutive hours. Must be able to walk 6 total hours per day and up to 10 total consecutive minutes. Must have the ability for hearing as it relates to normal conversation, high frequency and low frequency hearing sounds. Must have the ability for seeing as it relates to general vision, near vision, color vision and peripheral vision.
EDUCATION: Bachelor's Degree required in the human services field with relevant course work required dependent upon department assigned .
TRAINING AND EXPERIENCE: Displays sound theoretical knowledge of psychopathology, addiction, behavioral health, the ASAM criteria, and knowledge of life domain needs assessment. Must possess and maintain a valid driver's license along with a driving record in compliance with St. Luke's Penn Foundation policy. WORK SCHEDULE: Monday-Friday days, with flexibility for early evening hours as needed. On-call rotation as scheduled.
