We care for older adults beginning to need some help around the house, seniors who want to continue living at home, individuals with disabilities, and medically fragile children. We encourage growth and education at New Horizons, offering scholarship opportunities and an annual $500 education credit—which you can use toward cooking classes, CPR certifications, wellness courses, and more.Responsibilities:Coordinate and monitor services, including tracking of client activities in relation to the client's care planOversee the maintenance of comprehensive client files and other documentation of all client encountersPerform home visits as neededReceive and process telephone calls and electronic communicationsAssist in development of home service plansEnsure Oregon Administrative Rules are followedQualifications:High School Diploma or GED equivalentValid Oregon Driver License with valid auto insurance2+ years in care management preferredPossess strong written, verbal, and presentation skills with diverse populationsAbility to multi-task and stay calm in a fast paced environment while ensuring attention to detailNew Horizons is an Equal Opportunity Employer and is committed to hiring a diverse workforce and sustaining an inclusive culture.
