Position Summary The Southern Care Coordination Manager coordinates programs that support the mission of the organization. The Manager is responsible for maintaining the principles and standards for Ryan White Title II funds set forth by the Human Resources and Services Administration and the Oregon Health Authority and ensuring contract compliance. This position will implement and evaluate new projects, regularly assess current initiatives and conduct quality improvement initiatives as appropriate. The Manager oversees the Southern Oregon Care Coordination, Pharmacist Consultation, Dental, and Insurance Access directly, including supervision of staff in those programs. The manager will be responsible for all department chart related policies and projects, including file maintenance, chart audit and storage. The manager is also responsible for building and strengthening relationships with community partners and funders. In addition, the Southern Care Coordination Manager will act as the Northern Care Coordination Manager, when needed including to approve staff time accounting and expense requests, and general management duties when the Care Coordination Manager is not available.


Essential Functions and Responsibilities General Management
Work with the Care Director to provide all aspects of program development and administration including:






· program visioning
· planning and evaluation;
· problem/issue identification,
· analysis, research and resolution;
· budget development and monitoring;
· policy development and revision






Organized program record keeping and reporting
Development and monitoring of an annual program plan
Participate in Management Team meetings
Provide information to the Care Director for the preparation of timely quarterly reports for the Board of Directors and progress/final reports for program contracts/grants


Staff Supervision & Support
Provide supervision, training and support for program employees and volunteers
Provide de-escalation support for clients in crisis and support staff in productively working with clients in crisis
Train and supervise employees and volunteers
Facilitate department meetings
Support department lead positions in their roles
Ensure consistent communication with offsite employees
Maintain and nurture relationships with key funders, policymakers, supporters and community influencers related to client services
Bridge building with those agencies where our mission overlaps


Client Care
Ensure that clients receive quality, competent and responsive care
Address client concerns and complaints appropriately
Ensure compliance with HIPAA and all related confidentiality/privacy legal requirements
Stay up-to-date with RWCA legislation and regulations pertaining to HIV case management
Stay up-to-date with HIV/AIDS disease and treatment issues


Equity and Quality
Support and champion the development and implementation of program equity goals
Ensure that staff can track data necessary to monitor progress toward equity goals and to identify disparities in care for PLWHA
Identify training and provide equity focused and culturally responsive supervision to staff
Support collection of client feedback on a regular basis and using that feedback to make program improvements
Ensure that staff concerns related to equity are addressed
Ensure that policies and procedures and consistently applied
Provide staff with opportunities to share input related to program equity goals


Required Qualifications At least three years of experience in delivery of care or services to vulnerable populations AND
At least three years of coordination, management or leadership experience
Experience with motivational interviewing, case management, harm reduction and client- centered service
Ability to openly communicate and create an environment where employees, clients and partners feel respected
Excellent supervision skills, including supervising a large diverse team with a varying level of experience
Budgeting, administrative, financial management abilities and experiences
Effective spokesperson – ability to negotiate and represent agency
Ability to build and maintain bridges between agencies
Detail-oriented & Self-motivated
Desire to work with diverse populations
Ability to be flexible and adaptable
Experience working with drug and alcohol, mental health issues
Excellent written and oral communication
Team player who is self-motivated, high-energy, enthusiastic
Experience with Word, Excel, and Access
Experience with electronic client records
Preferred familiarity with Ryan White Care Act (RWCA)
Valid Oregon driver's license, driving record sufficient to be covered by agency auto insurance policy, ability to transport self to job-related events, meetings and locations
TB test (provided at agency) and COVID Vaccine
Flexible hours required, including some evenings and weekends

Repetitive use of hands such as grasping, twisting/turning of wrists, and finger dexterity necessary to perform various clerical duties, such as typing and writing. Frequent use of arms such as pushing and pulling to open and close drawers and cabinets Ability to sit or stand for extended periods of time at an assigned station Ability to bend, stand, and reach to be able to transport clients and to occasionally lift and carry up to 25 pounds (harm reduction kits, naloxone kits, testing and education materials) Continuous clarity of vision at 20 inches or less for processing of paperwork
