Care Coordinator

Job summary
Facilitating admission intakes, treatment planning, discharge planning, and coordination of care for patients in crisis.Educating and informing patients about treatment consents, communication, and compliance.Collaborating with the treatment team to develop and implement individualized treatment plans.

Responsibilities
• Conducting thorough psychosocial assessments and diagnosis with patients in a timely manner.• Obtaining signatures from licensed therapists for assigned tasks.• Gathering collateral information to assess immediate treatment and discharge needs.• Coordinating care for patients and addressing service gaps.• Engaging, monitoring, and communicating with individuals on the treatment team.• Documenting all services and patient activities in medical records.• Maintaining knowledge of Contracted Health Plan system and funding.• Participating in supervision, education, and ongoing competency development.• Following up with the director about all cases weekly.• Performing other assigned duties.
Requirements
• Bachelor's Degree in Social Work or related behavioral health field OR• Associates Degree with at least 2 years of experience in an acute or transitional outpatient setting.• Master's Degree in behavioral health or related field preferred.• Bilingual (Spanish) preferred.
