About the Opportunity
We are currently seeking a dynamic, full-time Community Engagement Specialist who embodies empathy and creativity to redefine healthcare accessibility.
As a Community Engagement Specialist, you will be instrumental in directly connecting with and enrolling individuals facing Serious Mental Illness, Substance Use Disorder, homelessness, and other high medical needs. We believe in the transformative power of trust and relationships in bridging the gap for those who have historically lacked access to comprehensive healthcare.
This role is predominantly remote, with occasional on-site requirements.
Key Responsibilities
Identify, engage, and enroll individuals into the program through various channels, including virtual platforms and in-person interactions.
Cultivate trust and rapport with eligible members, acknowledging and addressing potential barriers to enrollment.
Educate partners about services, emphasizing the benefits of Enhanced Care Management.
Leverage community insights and exceptional customer service skills to facilitate enrollment and foster lasting relationships.
Facilitate seamless communication among internal teams and external partners.
Meet enrollment targets while aligning with our mission to enhance the wellbeing of underserved communities.
Manage the enrollment process efficiently, providing regular updates on progress and addressing any delays or challenges.
Demonstrate proficiency in utilizing relevant technology platforms.
Must-Have Qualifications
Reside in or near Merced, California.
Maintain reliable transportation for on-site engagements.
Possess a quiet, HIPAA-compliant, internet-connected workspace for remote operations.
Previous experience in a metric-driven environment.
Demonstrated commitment to community improvement.
Results-oriented mindset with a track record of exceeding performance metrics.
Bilingual proficiency in English and Spanish, with excellent communication skills in both languages.
Strong technical aptitude and familiarity with diverse technology platforms.

Experience in high-volume outreach and engagement initiatives.

Active certification as a Community Health Worker (or willingness to obtain certification).
