Springwell in seeking a specialized Case Manager for older adults with behavioral health needs. DESCRIPTION:
The Advocacy & Navigating Care in the Home with Ongoing Risks (ANCHOR) program helps older adults with behavioral health needs accept and maintain services to avoid institutionalization and homelessness. The program provides a high level of in-person and telephonic contact with consumers to overcome their barriers to accepting services through the state subsidized Home Care program. All contacts including comprehensive assessments will be documented in the electronic statewide database. This Case Manager will work closely with consumer's informal supports as appropriate, other Springwell staff including RN and Protective Services, and various community partners including medical providers and vendor agencies. QUALIFICATIONS: Bachelor's Degree or higher. Applicants without a Bachelor's Degree may be considered if: The applicant has an Associate's Degree and at least 3 years' experience working with elders or providing case management services; OR The applicant has completed at least 50% of coursework required for a Bachelor's Degree and agrees to complete the remaining coursework within a 3 year period. Experience working at risk individuals due to behavioral health diagnoses, substance use, and/or homelessness preferred Experience working with computers, entering data, and running reports. Applicant must have a reliable car, current driver's license, and safe driving record.
