Job Summary: 
Direct Care workers specialize in providing support and assistance to clients in the comfort of 
their own home. As a Direct Care worker, you play a vital role in enhancing the quality of life for 
individuals who require personalized care and support to maintain their independence and wellbeing. The primary responsibilities are to provide compassionate care, perform daily living 
activities, and offer emotional support to clients while promoting their safety, dignity, and 
respect.
Essential Duties and Responsibilities:  Direct Care: Cares for individuals and families during periods of incapacitation, family disruption, and convalescence; providing companionship, personal care, and help in adjusting to new lifestyles. Administers bedside and personal care such as ambulation, transfers and personal hygiene assistance. Performs housekeeping duties such as cooking, cleaning, washing clothes and dishes, and running errands.  Assists with routine bodily functions, including eating, feeding, toileting, bathing, dressing, personal hygiene, grooming. Provides health maintenance activities including activities such as skin care, mouth care, medication reminders. Plans, shops for, and prepares nutritious meals, and assists families in planning, shopping for, or preparing nutritious meals and in accordance with prescribed diets. Instructs or advises clients on issues such as household cleanliness, utilities, hygiene, nutrition, or infant care. Entertains, converses with, or reads aloud to patients to keep them mentally healthy and alert. Prepares and maintains accurate records of client progress and services performed, reporting changes in client condition to manager or supervisor. Administrative: Becomes familiar with policies and procedures associated with the company.  Must accurately use the state and/or company mandated Electronic Visit Verification (EVV) system to track time & attendance and document visit activities Establishes an effective working relationship with management, families, consumers, and payor sources. Attends mandatory in-service training as directed.  Carries out all duties in a responsible, professional, and ethical manner upholding the mission of the Company. Performs other duties and activities as assigned. Knowledge, Skills, and Abilities: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Understanding of basic concepts of personal care, housekeeping and other care areas. Willingness to learn and demonstrate empathy towards individuals with diverse needs and backgrounds.  Must possess a strong service orientation, including critical thinking skills, and close monitoring of clients. Excellent verbal and written communication skills, with the ability to actively listen, understand and respond effectively to clients' needs and concerns.  Ability to meet physical demands of the job, including assistant with lifting, transferring, and providing mobility support to clients.  Maintain strict confidentiality and adhere to professional standards and ethics, respecting the privacy of clients and their families.  ABOUT CARE AND HELP HOME CARE, LLC CARE AND HELP HOME CARE, LLC ("Company") provides non-skilled services to individuals in their homes or other independent living environments. These services include (but are not limited to) assistance with self-administration of medications, personal care such as assistance with personal hygiene, dressing, and feeding, homemaking such as assistance with household tasks housekeeping, shopping, meal planning and preparation, and transportation, companionship, respite care such as assistance and support provided to the family; and other non-skilled services. We conduct criminal background checks when required on all staff. We also conduct physicals and tuberculosis screenings for all direct care workers and office staff with direct consumer contact upon hiring and on a yearly basis. The direct care workers of the Company are required to complete a minimum of forty (40) hours of experience or practical training and a competency evaluation, or a state certification program. This training includes information regarding personal hygiene, safe transfer techniques, infection control, and basic nutrition. All direct care workers of our company also receive supervised practical training before having direct contact with a consumer or verification of prior experience.
