Job Summary: Coordinates administrative and program specific functions to support all MRC programs. Maintains documentation and assures communication occurs between MRC staff, persons served, Integrated Services of Kalamazoo, home care providers and community members. Maintains authorization list and associated duties. Processes incoming referrals and discharges. Creates, maintains and reviews case records and makes program recommendations.
Essential Duties and Responsibilities: Tracks authorizations for both case management and ancillary programs. Ensure all individuals served have authorizations for services provided. Collaborate with other agencies to ensure proper units are in place for each consumer. Appeal services denied by funding sources. Maintains all various data reporting. Assist with preparation for and necessary follow-up for CARF, NISH, and QMR audits. Reviews case records to ensure standards and requirements are met. Create and maintain all case records, ensuring they are organized in a neat, consistent manner so that information can be found quickly and easily. Purge files as needed. Complete incident reports in accordance with agency policy. Compiles new client intake/annual packets for all programs. Uploads documents to the electronic health record and saves records in shared drives. Ensure program plans are in-serviced and track that training has occurred. Provide support to program staff to assure consumer needs are met. Collects and enters all initial and updated client demographic information into electronic health record and Excel database. Generates reports and provides this information for semi-annual reporting purposes. Processes referrals, discharges and transfers. Enters deductible information into electronic health records and coordinates with ISK Finance department. Coordinates transportation for persons served. Track necessary dates such as treatment plans, periodic reviews, court orders, referral dates, and engagement. Completes coordination of healthcare within the Electronic Healthcare Record. Provide training to all program staff on documentation best practices. Serve as liaison between funding sources and MRC, ensuring proper documentation is being shared and on file. Validate service codes and fees for services being provided to individuals. Collect, input into computer database, and report on assigned data including, but not limited to, consumer demographics, plans of service, attendance, unit records, and program evaluations. Connect and provide outside referral information to individuals and their families who request or need additional services which MRC does not provide. Complete staffing schedules monthly to ensure periodic reviews are being completed in a timely manner. Ensure all progress notes are completed correctly and changed over into file in a proper manner. Track, collect, and report on BMC data for assigned consumers. Provide in-service to all program staff on new individuals to McKercher services and/or changes to individuals' behavior treatment plans and intake meetings. Assist in collecting all key milestone numbers and provide this information to administrative assistant monthly to ensure that it is ready to be entered into semi-annual report Ensure all individuals are referred to MRS and receive necessary career counseling services for individuals receiving in-house skill-building services. Assist consumers in the completion of the MRS orientation process to ensure they are ready to receive job development services. Completes other miscellaneous duties as assigned. Criminal records, driving and recipient rights screening required. Valid Michigan driver's license, good driving record and the availability of licensed, insured vehicle for use on the job required. Physical Requirements: Ability to operate a computer keyboard at acceptable levels of quality and speed and other general office equipment as needed to perform the essential duties of the job. Physical ability to work in front of and view a computer terminal for prolonged periods of time. Ability to lift and carry up to 50 pounds. Physical ability to work sitting for prolonged periods of time. Work Conditions: May be exposed to bloodborne pathogens, infectious diseases, and parasites.
