Full job description The Case Manager is a grant-funded position that is renewable based on continuation of project funding, within the ARCHES Housing Opportunity Program (AHOP) of the Mid-Willamette Valley Community Action Agency's ARCHES Project. AHOP serves individuals and families in Marion and Polk Counties who are homeless or at imminent risk of homelessness.
This program is designed for moderate to high needs clients who are best supported through an extended level of assistance, while removing barriers that facilitate stable housing. The position will perform eligibility screening, program enrollment, participant follow-ups, process required paperwork, data entry and other general program supports. This position will directly align with the Housing First initiatives of the ARCHES Project.
Candidates must submit applications by 05/20/24 in order to be considered for first-round interviews. MINIMUM QUALIFICATIONS The requirements listed below are representative of the knowledge, skill, and/or ability required.
EDUCATION and/or EXPERIENCE A High School Diploma or GED and two years of related social service experience; or an equivalent combination of education and experience.
Ideal candidates will possess two or more years of homeless service delivery experience.
Preference will be given to applicants with knowledge of/or experience with: the principles of coordinated assessment, working with unsheltered or vulnerable populations, as well as best practices in homeless service delivery.
KNOWLEDGE, SKILLS & ABILITIES Basic proficiencies in computers, and MS Office products, database software and web tools. Must possess excellent planning, organization and time management skills. Ability to effectively communicate both orally and in written form.
Bilingual English/Spanish capabilities are required.
To qualify as bilingual, staff must possess demonstrated abilities to speak, write, and read fluently in Spanish and English and pass a bilingual test. The posted pay rate is inclusive of a 2% bilingual differential.

CERTIFICATES, LICENSES, REGISTRATIONS Driver Qualification Status is required for this position, including a valid driver's license, auto insurance, vehicle for use on the job or access to agency vehicle, and a satisfactory drive record for the previous five years.
Candidate must pass a comprehensive MWVCAA background screening prior to employment.
Candidate must pass pre-employment and random drug/alcohol screenings.
ESSENTIAL DUTIES AND RESPONSIBILITIES This job description is not intended to be all-inclusive. Employee will also perform other reasonably related business duties as assigned by immediate supervisor and other management as required.
Facilitates the distribution of homeless diversion, rapid re-housing, and barrier removal funds. Ensure access to housing assessments and resources.
Assesses client eligibility and aids in gathering documentation as required for intake, enrollment, and financial assistance.
Provides direct service to participants, including case management and housing placement support to assist participants in becoming self-sufficient and overcoming barriers to accessing permanent housing.
Develops Action Plans that identify participant needs, barriers, and strengths.
Establishes and maintains an active working relationship with community partners.
Provides support for client-tracking, service delivery, follow-up reporting, maintaining strict confidentiality.
Maintains full compliance with all federal, state, and grant program and funding requirements, setting high standards for data quality and compliance performance, remaining in compliance with the regulations of all funding streams.

SUPERVISORY RESPONSIBILITIES There are no supervisory duties in this position.

Occasionally operates a motor vehicle as needed to perform duties at sites or client locations.
Frequently lift up to 25 pounds. Occasionally lift up to 50 pounds.
Manual dexterity for handling computers, devices and office equipment
WORK ENVIRONMENT This position requires on-site work and is not eligible for remote work.
Occasional urgent situations requiring law enforcement involvement and/or paramedic professionals.
Close quarters, often with a client population experiencing homelessness, substance us disorders, and/or severe and persistent mental illness, and/or unpredictable behavior.
Occasional exposure to trash, bodily fluids and malodorous air.
Indoor work environment with frequent interruptions and demands. Occasional outdoor environment.
