This role demands a highly motivated and empathetic individual, capable of multitasking in a dynamic environment and committed to improving the health and wellbeing of local communities.YOUR OPPORTUNITIESEducational and Supportive ResponsibilitiesEducating members on preventative care, particularly in behavioral health and mental wellnessVirtually or in-person support for members' health needs, including short-term interventions and collaborative care managementConducting proactive outreach and maintaining engagement with members, especially those with high needs, to help them achieve their health goalsDocumenting member interactions and organizing this information efficientlyReferral and Assessment ResponsibilitiesMaking necessary referrals to the care team for additional medical or social resourcesConducting thorough needs assessments with patients in various settings and assisting with health-related social issuesHelping patients set and reach health goals, addressing social determinants of health, and assisting in patient coachingCoordination and Community Outreach ResponsibilitiesCreating and updating a directory of local community resourcesManaging relationships with healthcare facility providers and working closely with the care teamExpanding knowledge of community resources and building relationships with these organizationsServing as a liaison between services, aiding in care coordination, and advocating for clientsAttending community events for outreach and educationTechnology and Documentation Skills ResponsibilitiesTeaching patients how to use the Nuna app and providing critical feedback from the field to the Nuna product team Navigating various technology systems for detailed and accurate documentation of patient encountersSupporting both in-person and virtual patient interactions through phone or video visitsQUALIFICATIONSRequired QualificationsMaster's Degree in Social Work, Public Health, or equivalent experienceProficiency in using technology for remote support and comfortable with virtual communication platformsStrong problem-solving skills with a creative approach to healthcare challengesAdaptability to change, excellent organizational skills, ability to work independentlyStrong communication skills, cultural competency, and a commitment to serving diverse and underserved populationsFamiliarity with the Greater Chicago area and its community resources with residence within commutable distance of Chicago and willingness to travel within surrounding countiesValid driver's license and insured vehicle accessPreferred QualificationsCommunity Health Worker certificationExperience with home visits, managed care patients, and roles in customer or client serviceKnowledge of Chicago Medicaid populationsWe take into account an individual's qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company's equity program, paid time off, including vacation and sick leave. The expected salary range for this position is $72,000 to $85,500.
